Our service
We operate an open door policy - Carers can contact us direct or be referred by someone working in health, social care or the voluntary sector.
We support all adult carers even if they or the person they care for is not receiving statutory services.
We offer Information, Advice, Advocacy and Emotional support to adult carers.
We support carers by Telephone, Outreach Surgeries, and Home Visits.
Putting carers first - Our service focuses on carers, seeing things from their point of view and those they care for.
In all our work we will aim to empower individual carers to take control of their lives, play a full part in the local community and to move on from their role when appropriate.
Where we have identified needs that are not best met by our organisation, we will work pro-actively with other organisations to ensure that appropriate and timely services are provided.

For professionals
DPFC has a dedicated help line for carers that is also available to professionals who wish to access advice and information on topics relating to carers issues.
We are very happy to support people whose role involves working with carers to ensure carers are fully supported and recognised for the very important job they do – which can be demanding both emotionally and physically.

Referrals
We will welcome any referrals you wish to make on behalf of a carer living in the Doncaster area. Once carers have made contact with DPFC there is information, advice and support services available to help them in their caring role.
Either telephone or download a Professionals Referral Form (new window) and return the completed form by posting, faxing or emailing (see details below).
If you choose not to use the referral form, we will need the following information from you:

The carers name and address

The carers contact telephone number
-
Confirmation that you obtained permission from the carer for you to pass their details on to us
-
Your details i.e. where you work and a direct line for us to contact you if we have any queries
-
Reason for referral.

Presentations
We are also happy to give a talk / presentation about our work to your staff.
